The application from in two pages is published in Employment News as a part of the Employment Notification. It is given in 1:1 form so that it can be directly photocopied in A4 size paper to use as blank application form. The proforma of other certificates like vision test, caste certificate, EBC certificates etc. are also published along with the Notification so that same can be followed for production while applying for a post. Read the employment notice carefully to know the rules and regulations, eligibility criteria etc. before filling-up the application form. Attach copies of all the required certificates duly attested by Gazetted officer. Ensure that proper fee in the form of IPO or Demand Draft is submitted along with the application. The Demand Draft should be from a Nationalized Bank with a validity of not less than 6 months. The IPO purchase date should not be prior to the date of notification.
